Only service issues listed on this form will be addressed and any additional issues may be treated as a separate service call and may involve extra charges. The quoted rate which includes up to the first hour is the minimum charge per visit and is a flat rate and not dependent upon time on site and includes up to 1 hour of labour on site. For example: If we come to your house and after 5 minutes diagnose that you need a new part, that service call is $199.99 (including the first hour and 134.99/hr thereafter--excluding any applicable distance charges) and if we come back the next day and install the part the service call for the installation is an additional $199.99 (including the first hour and 134.99/hr thereafter--excluding any applicable distance charges)
